The Town of Mount Hope Permit Information
Dear Permit Applicant:
New York State is now requiring written proof of worker's compensation and disability insurance for all contractors working on your project.Form WC/DB 100 must be completed and sent to the state for verification and signing and when returned it must be attached to your building permit application. IF THE PROPER COMPLETED FORM IS NOT ATTACHED,YOUR PERMIT WILL NOT BE REVIEWED AND WILL BE RETURNED WITH NO ACTION TAKEN.
OR
Form BP-1 form affidavit for exemption of insurance for 1,2,3,4 family owner occupied residence's must be completed, notarized and appropriate documents attached and to the permit application papers. IF YOU ARE CLAIMING TO BE SELF-PERFORMING ALL WORK ON YOUR PERMIT THIS FORM MUST BE ATTACHED TO APPLICATION. IF COMPLETED FORM IS NOT ATTACHED YOUR PERMIT WILL NOT BE REVIEWED AND WILL BE RETURNED WITH NO ACTION TAKEN.**

**Please note that these forms are signed under the penalty of perjury.It is our understanding that if someone hires people to work on their project, contractor or not and the hours total more than 40 in any week for all those hired and you have failed to file the proper forms, you may be be held liable for any injuries occurring on your project and possibly the insurance costs and legal fees as well. Please be careful.
Any permits applied for on a county road must have this note on the site plan! NO EXCEPTIONS WILL BE MADE!
NO site preparation or construction shall commence until a valid entrance permit has been secured from the orange county department of public works under section 136 of the highway law!!!
